  • Employment Forms

    During this program, you will be guided through forms essential to beginning your employment at Savannah State University. You will receive an explanation of and brief instructions to complete the forms. Some forms are necessary to receive your paycheck.

  • SSU Right to Know Training

    The University System of Georgia has developed a plan to assure that all employees receive training and information about hazardous chemicals present in their work areas.

    Click on the "Right to Know Training" link below for the University System of Georgia Right to Know Training. At the end of the training, please complete the online form and print the certificate of completion. Note: If you are off campus, you will probably not be able to print the online certificate. If this is the case, please print out the page at the end of the training that states, "Congratulations! You have completed the Right-to Know Online Training program."

    Either that page or the certificate MUST be brought to your Human Resources session to verify your "Right to Know Training." After printing the certificate, close the window to return to this page, & click on the orange arrow to continue your orientation

  • Employment Policies

    It is your responsibility, as employees of the University, to become familiar with the policies that govern our conducts

    Please READ and PRINT each of the following policy statements. You will then print a form to sign, stating that you have received or read copies of the policies. This form must be brought with your other paperwork to the Human Resources session.

  • Drug and Alcohol Policy

    In compliance with the Drug-Free Schools and Communities Act Amendments of 1989, SSU has had in place, since the fall quarter of 1990, a formal policy for the faculty, staff, and students detailing standards of conduct and applicable sanctions related to the unlawful possession, use, or distribution of illicit drugs and/or alcohol by employees and students. In accordance with federal legislation, the university hereby notifies the faculty and staff that the unlawful possession, manufacture, distribution, dispensation and use of drugs and/or alcohol is prohibited and will not be tolerated on the campus os SSU. Members of the faculty and staff who unlawfully possess, use, or distribute drugs and/or alcohol on university premises or at any university sponsored activity will be subject to strong disciplinary action consistent with local, state, and federal law up to and including expulsion and referral to prosecution. The institution may also impose other sanctions on students consistent with appropriate regulations as dictated in The Policy Manual of the Board of Regents and other related policies and procedures.     Sexual harassment is no laughing matter.

    Wonder if a behavior is inappropriate or unwelcome?

    Just don't do it!

    SSU is committed to maintaining a fair and respectful environment for living, working, and studying. Reports of sexual harassment will be met with appropriate disciplinary action, up to and including dismissal from the University. SSU's Judicial Affairs Officer is the Harassment Prevention Officer. The University encourages any person who feels he or she has been harassed to report the incident to.

  • Tuition Assistance Program Policy

    The Board of Regents of the University System of Georgia and Savannah State University are committed to the need for the continued professional growth and development of its personnel. It considers support for educational expenses of personnel an important vehicle for addressing that need. The Tuition Assistance Program Policy is designed to foster the professional growth and development of the University System's non-temporary full-time employees.

    This program is offered subject to space being available in the course and funds being available within the institution. A non-temporary full-time employee at a University System of Georgia institution who is eligible for benefits and who has been employed by a University System of Georgia institution for at least six months may, upon the approval of the employee's supervisor or other designated official, be eligible to participate. When an employee is required to attend certain educational programs or when completion of certain educational programs is mandatory for retaining their current position, this policy does not apply.

  • Personal Data Form

    This form is to provide information needed to enter an employee into our personnel/payroll system. The University System of Georgia uses some of the demographic data for statistical purposes. This form will be retained in the employee's personnel file.

    This form is to provide information needed to create an email and network account. You will be required to sign the document electronically. A verification email will be sent to you to verify your signature. Once account is created, ITS will contact you with account information.

  • Employment Eligibility Verification

    The Immigration Reform and Control Act requires that all newly employed individuals provide documents which establish identity and employment eligibility and complete an I-9 form.

    You should complete Section I of the I-9 form and turn it in, along with the rest of the required employment forms, to Human Resources. You will be required to provide proper identification, so please read the accompanying instructions when completing the form.

    The University will employ only U.S. citizens and aliens authorized to work in the United States. Failure to comply with the provisions of the act will result in immediate termination of employment.

    Note: The I-9 form is a 3-page form. To avoid printing problems, please print the form one page at a time using the print button on the Adobe Acrobat toolbar.

  • Security Questionnaire - Loyalty Oath

    Security Questionnaire

    As a condition of employment, every employee of the University must complete and sign a security questionnaire.

    This is a 2-page form. You must complete the appropriate areas on both pages.

    Note: This form must be notarized in two places. Do not sign until you are in the presence of a notary.

    Loyalty Oath

    The State of Georgia requires "all persons who are employed by and are on the payroll of ...the State of Georgia, or its departments and agencies" to take a loyalty oath.

    If a person does not sign the oath, the act then instructs that "(the person) be taken from the payroll and not be permitted to receive any payment from the State."

  • Income Tax Withholding

    W - 4 & G - 4 Tax Forms

    All new employees should sign a W-4 Form (federal withholding) and a G-4 Form (state withholding) when they start work. The forms will be effective with the first wage payment. If you do not provide the University with completed federal and state tax withholding forms, taxes will be withheld at the maximum tax rate. Non-resident aliens may be subject to other requirements.

    Note: If you have been previously employed at SSU, and have completed tax forms on file with the Human Resources Office and wish to make no changes, you need not complete any additional tax forms. You may call Human Resources to see if your tax forms are still on file.

    USG Direct Deposit Notification Form

    Direct deposit is a condition of employment for all University employees. Your paycheck can be split between multiple accounts.

    In order to prepare the direct deposit form, you will need to provide the following information:

    • Exact name as it appears on your bank account
    • Account number
    • Name of bank
    • Voided check for checking account to attach to the form
    • Voided deposit slip for savings account

    The banking industry requires a pre-notification process that takes approximately three weeks to complete. As a result of this process, your direct deposit may not be effective until the second payday after your form is received in Payroll. Thus you may not receive a check until the direct deposit process has been completed.
